Reasons:
1) The temperature that day was too cold for the Shuttle.
2) The O-ring wasn't composed correctly.
3) Gas from the Shuttle began to leak.
4) The booster wasn't completely sealed which caused the leaking.
Solutions:
- If they had fixed the O-ring instead of ignoring the fact that there was something wrong with it, then the shuttle wouldn't have exploded.
- If the shuttle was made for extreme heat and cold, then it would have been able to withstand the temperatures no matter what it was.
- If the booster was sealed like it was supposed to then they'd be no leaking which would lead to no over heating and explosion.
